The questions in this survey are about fatigue and the effect of fatigue on your activities. You’ll find this questionnaire on pages 5-8 of your companion guide.

This first part of the course is all about establishing baseline measurements. I want you to have a number that identifies just how tired you are.
At the end of the course, when you’ve identified exactly which foods cause your fatigue (and you’ve eliminated them) you’re going to re-take these tests to see just how much your fatigue has improved!
