Check your email inbox! You should have the newest fatigue questionnaire. It’s called the Checklist For Individual Strengths (CIS).
You’ll find this questionnaire on pages 15-18 of your companion guide.
The CIR consists of 20 statements. With each of these statements, please record the impression of how you have felt during the past two weeks.
For example, let’s use the statement:
I Feel Relaxed
If you feel that this statement is not true at all, place an “X” in the far right box.
If you feel this statement is very true, place an “X” in the far left box.
If you feel that a statement is not “yes, this is true” or, “no, this is not true”, place an “x” in the box that is most in accordance with how you have felt.
